LIXI MEMBERSHIP AND LICENSING APPLICATION

Congratulations on your decision to join LIXI. Application forms for Licensing and Membership are available for download below.  Should you wish to discuss your membership needs further, please email admin@Lixi.org.au or complete your contact details here and someone will be in touch with you shortly. 

Please read all the required documents carefully, complete all required forms and return via post to the address shown or scan and send to admin@lixi.org.au Upon receipt of signed application forms, LIXI will be in touch in regards to your application and how your application is progressing.

Membership Forms
To become a LIXI Member, please complete the Membership Application Form here, read and sign the Membership Terms & Conditions and read and initial the Membership IP Policy.

Licensing Forms
To become a LIXI Licensee, please complete the LIXI Licensing Application Form and read and initial the Licensee End User Agreement.
